All NCH hospitals have a Care Management Team, that provides supportive health and social services to help address the emotional stress and uncertainty resulting from your illness or hospitalization. Care Management assistance can be obtained by asking your doctor or nurse. These services are confidential:

Advanced Directives
Transition Planning
Long Term Care Planning
Promoting skilled nursing referrals
Collaboration with Primary Care Physicians & Community Agencies
Outreach Education
Liaison between patients, families and care providers
