The MyAVH Patient Health Portal gives patients instant access to their health information, including laboratory results, medications, allergies, demographic information, visit history, discharge/aftercare information, and more. Parents have portal access to medical records for children ages 0-11 years.

When you reach the enrollment screen, please fill in your name (all capital letters), date of birth, Medical Record Number or Social Security Number, and e-mail address. If choosing the Medical Record Number option and you do not know what it is, you may call (603) 326-5826 to request it.
To sign up by phone call, you may call (603) 326-5826, Monday through Friday, 8 a.m. to 5 p.m.
